Benign Paroxysmal Positional Vertigo

BPPV is the most common cause of vertigo. Tiny calcium carbonate crystals (otoconia) dislodge from the utricle and migrate into the semicircular canals, sending false rotational signals to the brain. Not all physiotherapists are trained to deliver it.

The treatment itself is not painful. Repositioning manoeuvres for BPPV may temporarily trigger brief dizziness during the session as the crystals are guided back into place — but this is expected and subsides quickly. Mild exacerbation of dizziness following vestibular exercises is normal and is actually a sign the brain is adapting. Your physiotherapist will provide clear guidance on what is expected versus what warrants contact. You will also receive a home exercise programme with safety instructions. If you experience sudden severe vertigo, new neurological symptoms (weakness, slurred speech, double vision), or a fall, seek urgent medical attention.
